- Letter of intent as outlined on the program MAP which can be found on the Exercise Sciences website under Grad Students
- Minimum GPA of 3.2 for last 60 semester hours of upper level academic work
- Resume
- Applicants desiring the Master of Athletic Training (MAT) must submit supplementary application documents (Technical Standards, Physical Exam, Immunization/Background Check/Drug Screening) and must be ECC (CPR-AED) certified before first semester of MAT
- Non-native English speakers who are not eligible for an exemption must obtain the minimum required score on any of the English proficiency exams accepted by BYU Graduate Studies.
