- Resume/CV: A minimum of three years of full-time work experience is required. Experience does not have to be clinical or in public health
- Official Transcripts: A bachelor’s degree with at least a 3.0 GPA
- Personal Statement/Statement of Purpose
- Three Letters of Recommendation
- GRE Scores: The GRE (Graduate Record Examinations) requirement can be waived for applicants who have at least a 3.0 GPA. A recent GRE score (within the last 5 years) is required for those who have a GPA lower than the required 3.0.
- TOEFL/IELTS Scores (international applicants only): Test of English as a Foreign Language (TOEFL) or the International English Language Testing System (IELTS) is required for applicants whose native language is not English, or who have completed post-secondary education outside of the U.S.
